Services

Interested in attending a Day Program or moving into one of the Residential Programs in our area? Here's what you need to do:

  • First - Contact your Service Coordinator and inform them of your need or desire to attend a Day Program or move into a residence. (How do I get a Service Coordinator?)
  • Second- Your Service Coordinator will discuss your needs or desires. Your support plan will be amended, if necessary. Your Service Coordinator will offer you a choice of providers from the (Qualified Provider List).
  • Third - Once you chosen your provider, your Service Coordinator will contact the provider and work to have you enrolled.

Remember, Services are based on need and availability of resources. A consumer could be in need of many services and supports. However, it may not be possible to receive the service or support needed because of waiting lists or funding constraints.
